Blood Tests: Performed by healthcare providers, and can detect pregnancy earlier and measure exact hormone levels.

2. Missed Menstrual Period

For many, pregnancy suspicion often begins with a missed period. Pregnancy disrupts the usual hormonal cycle that causes menstruation, and a missed period is often the earliest noticeable sign. The Most Common Methods Used to Estimate Pregnancy Dates

1) Last Menstrual Period (LMP) Method

This method is used because it's simple, quick, and provides a consistent baseline. It also aligns with how pregnancy is commonly tracked: 40 weeks from LMP (on average).

When it may be less accurate:

  • If you have irregular periods.
  • If you don't remember the date well.
  • If you recently stopped hormonal birth control and your cycle is still adjusting.

2) Early Ultrasound Dating (Often the Most Accurate)

An early ultrasound is often considered the best way to estimate the due date, especially in the first trimester. In early pregnancy, the embryo/foetus grows at a fairly predictable rate, and the ultrasound measures size (often called crown-rump length early on) to estimate gestational age. This method is helpful because it's particularly useful when LMP is uncertain or cycles are irregular, and it can also confirm or adjust the estimated due date.

Best timing:

Generally, the earlier the ultrasound (especially in the first trimester), the more reliable it is for dating.

3) Conception Date / Ovulation Date (If Known)

This method uses the date you likely ovulated or conceived, but it's only accurate if that date is reasonably certain. If you know when you ovulated, pregnancy can be estimated from that point. And ovulation typically occurs about 14 days before the next period in a "textbook" cycle, but real cycles vary widely. This method is useful if you were tracking ovulation with ovulation predictor kits (OPKs), basal body temperature (BBT) charts, and detailed fertility tracking. However, even if you know when sex happened, conception may not occur that same day because sperm can survive for several days and ovulation timing can shift.

4) IVF or Fertility Treatment Dating (Most Exact)

If pregnancy occurs through IVF or specific fertility treatments, dating can be highly precise because key dates are documented. This method works by providers using the embryo transfer date and the embryo's age (e.g., a 5-day embryo transfer), and this provides a strong estimate of gestational age and due date. This method is accurate because the timeline is based on known medical dates, not estimates.

5) Physical Exam and Uterus Size (Fundal Height)

As pregnancy progresses, providers may estimate fetal growth by measuring the uterus. A provider measures the uterus from the pubic bone to the top (fundal height), and after a certain point in pregnancy, fundal height in centimetres often roughly matches weeks pregnant (with some normal variation). This method is best for tracking growth trends over time, not setting an initial due date, but it's less reliable for dating body shape, baby position, fluid levels, and multiple pregnancies can affect measurements.

6) "Quickening" (First Felt Baby Movement)

This is the moment someone first notices foetal movement. Often felt sometime around the middle of pregnancy, but it varies widely. This method is not used as a primary dating method as feeling movement depends on many factors, including whether it's a first pregnancy, placenta position, body sensitivity, and the baby's activity. And it's meaningful but not precise enough to accurately date a pregnancy.

7) Hormone Levels (hCG) in Early Pregnancy

Blood tests can track hCG, a hormone that rises early in pregnancy. Rising hCG supports the idea that a pregnancy is progressing early on, and sometimes it helps estimate whether a pregnancy is very early or further along.

Limits:

  • hCG ranges vary significantly between individuals.
  • It's usually not accurate enough on its own to assign a reliable due date; ultrasound is better for that.
